- What Insurance Do You File?
We currently accept BCBS, Aetna, Tricare, Humana, Viva, Medicaid
and United Health Care along with several others. Insurance
contracting can change so please contact us to verify that we
still accept your insurance prior to booking an appointment.

- Does my child need to see a physician to complete camp & sports physical forms?
Your child should have had a physical within one year of the date
the form needs to be completed. If you are unsure please contact us.
We need at least 48 hours to complete most forms.
We ask that you complete all portions of the form that you can prior
to leaving them with our receptionist.
- Can my babysitter or relative bring my child for office visits?
New patients must be accompanied by a parent for the initial visit.
Established patients may be brought in by anyone who is listed on
the protected health information form in your chart. If you must
send them with someone who is not listed in the chart please send
a signed "permission to seek appropriate medical attention" letter
with the person who will be bringing your child to the office.
